This application was written as a "paperless office" approach to create certain forms. Currently, this application allows users to create Request Form documents, savable in PDF format, from values submitted via a web-based form. Initially, there is only a (PO) Request form, however, additional forms may be added in the future.

Getting Started

  1. Click the Add PO Request button on the right side of the screen.
  2. Fill in the desired fields, additional item rows may be added (up to 15 lines).
  3. Click Submit
  4. If errors are detected, correct the errors and re-submit the form.
  5. Verify the details you submitted, if you need to change something you can click Edit.
  6. Click View/Save PDF to view, save and or print the PDF document.

Sounds simple, right? Give it a try.

Additional Information

  • To create a new Request form using the same Payable To fields, click the plus [+] on the lower right side of the Payable To box on the Request Details page.
  • Forms created by Guest users will be deleted after 48 hours. To save forms, create a user account. Forms created by registered users are not automatically deleted.
